29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. a) Jobsite, seventh floor wall opening on front side of building, 600 Block of Washington Road, Pittsburgh, Pennsylvania 15228: Observed on or about March 20, 2013, employees working near an unguarded wall opening on the seventh floor of a multistory structure were not protected from falling. The employees were transferring boxes of windows from the basket of an articulating boom aerial lift to the seventh floor of the building. The employees were exposed to a fall hazard of approximately 62 feet 8 inches to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: a) Jobsite, seventh floor wall opening on front side of building, 600 Block of Washington Road, Pittsburgh, Pennsylvania 15228: Observed on or about March 20, 2013, employees working near an unguarded wall opening on the seventh floor of a multistory structure were not protected from falling. The employer did not train employees in the recognition of fall hazards and the methods and procedures required to reduce or eliminate these hazards. The employees were exposed to a fall hazard of approximately 62 feet 8 inches to the ground below.
