29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a) Machine "VM2": On March 21, 2013 and at times prior thereto, employees were exposed to amputation hazards from the milling machine due to the disabling (bypass) of the interlock on the door of the machine.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:       a) Machine Shop: On March 21, 2013 and at times prior thereto, Employees were using approximately 55 p.s.i. of compressed air to clean the inside of the milling machines. This exposed employees to struck by hazards and compressed air injected through the skin and into the body.
