The horizontal lifelines were not designed, installed, and used under the supervision of a qualified person. Two employees who were tied off to the same horizontal lifeline with retractable lanyards; they fell 20 feet and landed on lower level in an accident. The horizontal lifeline deflected and the fall arresting mechanism of the retractable lanyards did not trigger to save employees from fall. On or about 03/18/13.    NOTE:  IN ADDITION TO CERTIFICATION OF ABATEMENT, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
