29 CFR 1926.502(b)(13):     When guardrail systems were used around access points, they were not provided with a gate, or were not so offset that a person could not walk directly into the hole:    (a) Location - 17-21 Ocean Avenue, Lynn MA 01905    On or about 03/25/2013, employees were exposed to a fall from a ladder-way that was not protected on all open sides.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Location - 17-21 Ocean Avenue, Lynn, MA:    On or about 03/25/2013, employees were not protected from falling while using a ladder that did not extend at least three feet above the upper landing surface.
