29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:    (a) Walk-in Freezer - Employees were required to enter the walk in freezer to get frozen food items.  There was a sheet of ice on the floor and an uneven walking surface in the freezer, due to mounds of solid ice and chunks of loose ice. Employees were exposed to a slipping tripping hazard on or about March 25, 2013.
