29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment is isolated from the energy source and rendered inoperative: On or about March 26, 2013, and at times prior thereto, employees were exposed to an amputation hazard where the employer did not establish an energy control program for equipment such as but not limited to the Wegoma automatic up cut mitre saw, Model CS-600, while maintenance is performed.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded: a) Production area, Jet turret milling machine, Model No. JTM-4VS, Serial No. 11919240. On or about March 26, 2013, and at times prior thereto, employees were exposed to an amputation hazard from the bit not being guarded, while milling aluminum extrusions, measuring approximately 1 1/2 - 2 1/4 inches wide by 24 - 60 inches long. b) Production area, Jet vertical band saw, Model VBS-1408, Serial No. 10051559, Stock No. 414483. On or about March 26, 2013, and at times prior thereto, employees were exposed to an amputation hazard from the saw blade not being guarded, while cutting 5/8-inch hexagonal shafts.
