29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.    a.  On or about March 26, 2013 - at the above addressed jobsite, an employee working in a SKYJACK SJ40T telescopic boom aerial lift was not attached to the boom or basket by the use of a lanyard, thereby exposing the employee to fall harzards of approximately twelve (12) feet three (3) inches.
