29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   (a)  On the overhang of the building:  On March 27, 2013 and at times prior to; the employer failed to provide eye and face protection to an employee while using a reciprocating saw to cut out sections of the overhang, exposing that employee to eye injuries.
