29 CFR 1926.453(b)(2)(iv):   Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:   a.  On or about March 22, 2013, at the south side of building No.3, an employee installing fascia elevated in an aerial basket of a JLG aerial lift was standing on the mid rail and the edge of the basket while working, exposing himself to a 30 feet fall hazard.

29 CFR 1926.453(b)(2)(v):  A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   a.  On or about March 22, 2013, at the south side of building No.3, two employees installing fascia were using a JLG 400S aerial lift without a personal restraint system with a lanyard fastened to the basket, exposing themselves to a 30 feet fall hazard.
