29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:        a) Duffin Manufacturing Company located on 316 Warden Street in Elyria, OH: On or about March 27, 2013, the employer failed to properly guard equipment such as, but not limited to the Bridgeport Drill Press in the maintenance area. Employees were not protected from inadvertent contact and were exposed to a rotating chuck during drill operations.     b) Duffin Manufacturing Company located on 316 Warden Street in Elyria, OH: On or about March 27, 2013, the employer failed to properly guard equipment such as but not limited to a Sharp metal lather in the maintenance area.  Employees were not protected from inadvertent contact and were exposed to a rotating chuck during machine operations.     c) Duffin Manufacturing Company located on 316 Warden Street in Elyria, OH: On or about March 27, 2013, the employer failed to properly guard equipment such as but not limited to the Brown & Sharp single spindle screw machine and prevent caught-in hazards created by a rotating tool turret during manufacturing operations.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked:  a) Duffin Manufacturing Company located on 316 Warden Street in Elyria, OH: On or about March 27, 2013,  the employer did not ensure that several unmarked five gallon containers of kerosene near the multiple spindle line were properly labeled to identify its contents.
