29 CFR 1910.212(b): Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving.    a.  Located in the middle of the production area, employees operated mechanical power press #561 while the press was not secured to the concrete floor, thereby exposing the employees to caught-between and/or struck-by hazards.       a. Located in the middle of the production area, employees operated mechanical power press #628, serial number 25121, while the press was not secured to the concrete floor, thereby exposing the employees to caught-between and/or struck-by hazards.

29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.    a.  Located in the production area, there were four electrical disconnect boxes that were used in part to start and stop electrical power to mechanical power presses.  The electrical disconnect was blocked by plastic crates of metal automotive material, which did not allow for sufficient access to the electrical disconnect if an emergency occurred with a power press and electrical power had to be immediately removed.
