Blackswamp Steel, Inc. worksite located at 1301 E. Mall Dr., Holland, Ohio: On or about March 27, 2013, the employer did not ensure an employee did not stand on the guardrails of a Genie Aerial Lift, model S-40. The employee was exposed to a potential fall while standing on the midrail of the lift performing connecting activities.

Mall Dr., Holland, Ohio: On or about March 27, 2013, the employer did not ensure an employee's personal fall arrest system was not secured (clipped-off) to a Genie S-40 Aerial Lift. The exposed employee was performing steel erection activities from the aforementioned aerial lift.

Blackswamp Steel, Inc. worksite located at 1301 E. Mall Dr., Holland, Ohio: On or about March 27, 2013, the employer did not ensure employees were protected from fall hazards while landing steel decking on the commercial structure. The exposed employees were exposed to a falll distance of approximately 17.00 feet to the level below.
