Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of struck-by:   On or about March 27, 2013, an employee was exposed to the hazards of being struck by the drilling mast.  The approximately 3/8 inch guy wire rigging cable used to keep the mast vertical was secured to an approximately 3 inch diameter branch of an approximately 10 foot tall tree.

29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:                                                                                          On or about March 27, 2013, an employee was exposed to a struck by hazard while using a steel alloy sling to rig drilling well production equipment.  The employee was observed using the approximately 15 foot long sling which did not have and identifying information affixed to it.
