29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a)  At the jobsite, on the roof, on or about 11/29/12:  Employees were working on a 4:12 pitch roof with a ground to eave height of 10 and 20 feet, without fall protection of any kind and using a 6 foot step ladder to access the upper roof surface that was not open with the spreaders locked.  Abatement certification must be submitted for this item.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  At the jobsite, on the roof, on or about 11/29/12:  Employees were working on a 4:12 pitch roof with a ground to eave height of 10 and 20 feet, performing roofing work that included stripping material and throwing it to the ground where an employee was working below without head protection.  Abatement certification must be submitted for this item.
