In the maintenance shop on the wall next to the entrance to the storage room there was an unused hole in the electrical box where there was a 120 volt receptacle mounted. Employees were exposed to a shock hazard.

In the truck parking shed on the back wall there was a broken old style 120 volt receptacle that had exposed conductors. Employees wre exposed to a shock hazard.

29 CFR 1904.32(b)(3): A company executive did not certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete. The person who certified the annual summary was not a company executive as defined in 29 CFR 1904.32(b)(4).   The OSHA 300 Log was not certified by a company executive for the years 2009 and 2010, and in 2011 it was not certified.
