29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On March 20, 2013 at the jobsite located at 11458 N Oak Trafficway in Kansas City, MO employees were exposed to a fall hazard in that they were working on a steep roof, approximately 18 feet above ground level, without an approved form of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about March 20, 2013, at the jobsite located at 11458 North Oak Trafficway, in Kansas City, MO employees were exposed to a fall hazard in that they were accessing and egressing the roof structure without the proper use of their ladder. In that the ladder side rails did not extend at least 3 feet above the eve of the roof, nor was the ladder secured at the top.
