Section 5 (a) (1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazard of being struck by fragments of plastic pipe due to the pipes potential for explosive shattering when used to transport compressed air.  On or about 28 March 2013, a compressed air system installed throughout the printing shop floor was using a 1/2" Schedule 40 polyvinyl chloride (PVC) pipe operating at approximately 125 psi. The plastic service line was used for various machines and tools, and had multiple drops to quick connect outlets for air nozzle lines.  PVC pipe and fittings do not possess shatter resistant properties and the brittle PVC material can be broken or shattered by external forces and stress.  This pipe is designed for transmittal of liquids which do not possess the high amounts of stored energy present in compressed gases. This hazard has been recognized by the manufacturer of this PVC product, the Plastic Pipe Institute, the American National Standards Institute, and OSHA through an official Safety Hazard Information Bulletin dated May 20, 1988.  Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standards for use of this PVC pipe, including the steps it is taking to ensure that piping used to transport compressed air and other gases is approved for the installation and service.

29 CFR 1910.242(b):   Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) (DESCRIBE HAZARD(S) WHERE NECESSARY)  On or about 28 March 2013, and at times prior thereto, employees were using compressed air at pressures of approximately 40 psi for cleaning purposes, where there was no safety relief devices found on the nozzles, exposing the employees to the hazard of being struck by flying objects.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure pressure relief devices are attached to the air nozzles to lower the venting pressure to at least 30 psi.

29 CFR 1910.147(c)(1):   The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about 7 March 2013, and at times prior thereto, the employer did not establish a lock-out and tag-out program for the Heidelberg Speedmaster Printing Press, Model SM 74-5-P, located in the southwest corner of the shop floor. Employees perform some replacement of parts for the press, which requires per the manufacturer's literature to be locked and tagged during so, to prevent either the release of stored energy to moving parts or exposure to potentially energized parts.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure a lock-out and tag-out program is established in the workplace.

29 CFR 1910.178(l)(1)(i):     The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph.    On or about 28 March 2013, and at times prior thereto, the employer did not implement a forklift training and evaluation program within the workplace to certify operators as being qualified and competent to operate the forklift, exposing the operators and other employees in the establishment to the hazard of being struck by either the forklift or loads carried by the forklift.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure operators are trained, evaluated, and certified on the safe operation of the forklift in the establishment.
