29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns  were not protected by protective helmets:  This violation occurred on April 01, 2013, at a work site located at 9205 Shoveler Trail, Fort Worth, Texas; employees performing decking work were exposed to struck by hazards from falling tools or materials and were not protected by the use of hard hat.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented.     This violation occurred on April 01, 2013, at a work site located 9205 Shoveler Trail, Fort Worth, Texas; an employee performing decking work from the roof, was not protected from falling more than 10-feet to the lower level by use of guardrail or personal fall arrest system.
