29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. (a) Jobsite: On or about March 31, 2013, an employee was not protected from falling approximately 30 feet while he was working from a pump jack scaffold. The employee was installing vinyl siding on a residential house.

The pitch of the roof was 8 in 12. The fall hazard to the employee was approximately 11 feet to the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Jobsite: On or about March 29, 2013, an employee was using a 22 foot extension ladder that only extended approximately 1 1/2 feet beyond the edge of the roof. The fall hazard to the employee was approximately 11 feet.
