29 CFR 1910.23(a)(8)(ii): While the floor hole cover of standard strength and construction is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing: a) On or before 3/5/2013, employees were climbing into the furnace unit and walking inside while floor holes which they could fall through, were unprotected by a cover. Abatement Certification and Documentation is required.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: a) On or before 3/5/2013, the company had identified a fall hazard for employees accessing and working within a dismantled 100 Ton furnace as part of relining operations. Employees climbing into the unit and walking within it while floor openings were exposed were not required to use an appropriate fall arrest system. Abatement Certification and Documentation is required.
