29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:       a)    On or about April 2, 2013, on the north side of the restaurant construction project, two employees working at an elevated height of 15 feet while on a platform on a Universal fabricated frame scaffold, were exposed to fall hazards.  The scaffolds legs were supported with hollow concrete blocks and loose boards stack on top of the blocks.  The employer did not initiate and maintain an accident prevention program to control and eliminate the hazards present at the jobsite in accordance with OSHA regulations.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):        a)    On or about April 2, 2013, on the north side of the restaurant construction project, two employees working at an elevated height of 15 feet while on a platform on a Universal fabricated frame scaffold, were exposed to fall hazards due to the scaffolds foundation not being stable.  The scaffolds legs were supported with hollow concrete blocks and loose boards stack on top of the blocks.  The employer did not initiate inspections by a competent person to control and eliminate the hazards present at the jobsite in accordance with OSHA regulations.
