29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate:  a)  At the worksit, 14441 Almeada Road, on April 2, 2013, the storage floor over the restrooms and auxillary offices was not equipped with a load limit.  This condition exposed employees to stuck-by hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  At the worksite, 14441 Almeda Road, Houston, Texas, on April 2, 2013, the chuck of the Mori Seiki lathe (model number SL-65, serial number 1333) was not completely guarded.  This condition exposed employees to caught-in hazards.

29 CFR 1910.305(a)(1)(i): Metal raceways, cable armor, and other metal enclosures for conductors were not connected to all boxes, fittings, and cabinets to provide effective electrical continuity:  a)  At the worksite, 14441 Almeada Road, Houston, Texas, the conduit leading to the 240 volt 30 amp knife switch attached to the LBlond Lathe was not connected.  This condition exposed employees to electrical shock, burns, and electrocution.

This condition exposed employees to electrical shock, burns, and electrocution.
