29 CFR 1926.453(b)(2)(iv): Employee did not stand firmly on the floor of the basket, climbed on the edge of the basket and used planks, ladders, or other devices for a work position: a) Metal canopy, East side of commercial buliding - On or about April 3, 2013, an employee fixing the trim of a metal canopy was exposed to a fall hazard from a height of approximately 10 feet while using the guardrails of a JLG Boom Lift basket (Model 450AJII, Serial Number: 0300158120) to gain access to and exit the top of the metal canopy. b) Metal canopy, East side of commercial buliding - On or about April 3, 2013, an employee fixing the trim of a metal canopy was exposed to a fall hazard from a height of approximately 10 feet while standing on the mid-rails and toeboards of a JLG Boom Lift (Model 450AJII, Serial Number: 0300158120).

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift: Metal canopy, East side of commercial buliding: On or about April 3, 2013, an employee performing sheet metal work on a metal side canopy at a height of approximately 10 feet above the landscape soil below, was exposed to a trip and fall hazard while standing in and driving an aerial lift without the use of a fall arrest system.
