29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about Wednesday April 3, 2013 at 3871 Peachtree Road Atlanta, GA 30345;  An employee removing concrete bricks from the top of a metal storage shed at about 8' 5" from the lower level was not wearing fall protection.

29 CFR 1926.602(c)(1)(vii): Unauthorized persons were allowed to ride on a powered industrial truck:  On or about Wednesday April 3, 2013 at 3871 Peachtree Road Atlanta, GA 30345;  The employee was observed being elevated to the top of a storage shed in the bucket of the bobcat that was being operated by the foreman to retrieve concrete bricks.
