29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about April 3, 2013 at 1320 N. Fairfield Road, Beavercreek, an electrician was exposed to an approximately eight foot fall hazard while working in a vehicle-mounted aerial llift basket without wearing any fall protection.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag shall be placed on each disconnecting means used to deenergize circuits and equipment on which work is to be performed, except as provided in paragraphs (b)(2)(iii)(C) and (b)(2)(iii)(E) of this section. The lock shall be attached so as to prevent persons from operating the disconnecting means unless they resort to undue force or the use of tools. On or about April 3, 2013, at 1320 N. Fairfield Road, Beavercreek, an electrician working in a vehicle mounted aerial lift basket was replacing a ballast, 110 Volt AC, for flourescent lighting located inside a sign and did not lock out the on-off switch to the sign.
