Certification is to include,but not be limited to, the name of the operator, the date of the training or the evaluation, the name of the person providing the training or evaluation, etc.    a- Max L. Brown Hardware Co., Inc., Carteret, NJ, Warehouse - Forklift training was not provided to those employees who are required to operate Forklift Trucks.    Condition noted on or about 4/3/2013.
