29 CFR 1910.23(a)(4): Skylights were not guarded by a standard skylight screen or a fixed standard railing on all exposed sides: On or about March 27, 2013 and at times prior thereto, employees were exposed to falls of approximately 26 feet in height to the concrete floor below, when accessing the roof and walking by unguarded skylights, while servicing the air conditioning units on the roof.

29 CFR 1910.106(d)(5)(i): "Egress." Flammable or combustible liquids, including stock for sale, were stored so as to limit use of exits, stairways, or areas normally used for the safe egress of people: On or about March 27, 2013 and at times prior thereto, employees were exposed to burns from the flammable materials such as but not limited to: Methyl Ethyl Ketone and Isopropyl Alcohol stored in the flammable storage cabinet located next to and by the stairway, when egressing the roof through the mezzanine, while conducting maintenance and servicing the air condition units on the roof.
