29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    At the time of the inspection, it was determined that at the time of the amputation injury on 3-5-2013 the Strickland Mixer was operated without guards to prevent exposure to rotating parts from the top of the mixer and the lower hinged opening.
