Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that are likely to cause burns, serious injury or death in that employees are exposed to fire and explosion hazards.    For the establishment at 10659 Rocket Blvd., Orlando, FL 32824, as observed on or about 04/04/2013:  a) A metal sander was present that generated aluminum metal dust. The aluminum metal dust was captured and transported through flexible duct to a dry cyclonic dust collector which captured the dust in fabric bags located on the outside of the collection vessel. The dust collector was located inside the building next to the sander and was not equipped with any deflagration panels or other explosion protection. All three fabric bags contained dust accumulation, and 1.4 pounds of dust was collected from one fabric bag. Employees working, walking and standing next to the dry dust collector were exposed to fire and explosion hazards associated with the accumulation and suspension of combustible aluminum metal dust in the dry dust collector located indoors.  b) The ducts between the sander and the dust collector were connected with duct tape, and there was an unused port for ducting on the dust collector that had been covered/blanked off with duct tape resulting in potential static buildup due to the use of nonconductive material.

29 CFR 1910.147(d)(3):  All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source:            For the Wrico Metal Stamping facility at 10659 Rocket Blvd., Orlando, FL 32824, as observed on or about 04/04/2013:     a) An employee on press 451 was observed to be working on press setup taking out bolts and clamps, and the press was fully energized. The main power had not been de-energized, nor had the safety block interlock been disconnected and the safety block inserted to guard against hazardous energy from the ram.  b) An employee on press 306 was observed to be working at the point of operation of the ram while the press was fully energized. The main power to the press had not been de-energized, nor had the safety block interlock been disconnected and the safety block inserted to guard against hazardous energy from the ram.
