Maintenance was performed on many of the CnC and other types of machines, and procedures were not developed to ensure safe energy isolation and lockout tagout of equipment during maintenance. Equipment worked on included 240V Fadal CnC Machines with 100 PSI air lines. Other types of metal working machines with hazardous energy consisting of 240V electricity and 100 PSI pneumatic air lines were also present on site.
