29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) For Wilsons Machine Products facility at 1844 Kentucky Ave., Winter Park, FL 32789, as observed on or about 04/05/2013: lockout tagout specific procedures were not available. Maintenance was performed on many of the CnC and other types of machines, and procedures were not developed to ensure safe energy isolation and lockout tagout of equipment during maintenance. Equipment worked on included 240V Fadal CnC Machines with 100 PSI air lines. Other types of metal working machines with hazardous energy consisting of 240V electricity and 100 PSI pneumatic air lines were also present on site.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) For Wilsons Machine Products facility at 1844 Kentucky Ave., Winter Park, FL 32789, as observed on or about 04/05/2013: employees used a bench grinder with its tongue guard missing, and a half inch gap between the peripheral guard and the wheel existed.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  a) For Wilsons Machine Products facility at 1844 Kentucky Ave., Winter Park, FL 32789, as observed on or about 04/05/2013, employees used compressed air at pressures of 80 p.s.i. dead front pressure to clean off parts/shavings.
