29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: Mountain Lake, Docking Area: On April 5, 2013, and at times prior thereto, employees were exposed to potential impalement hazards while gaining access to and from boat(s) and working near the vertically protruding uncovered t-bars and a wooden stake, none of which no longer served a function.

29 CFR 1926.251(a)(2)(i): Employers did not ensure that rigging equipment had permanently affixed and legible identification markings as prescribed by the manufacturer that indicate the recommended safe working load: (a) Mountain Lake, Presidio, SF: On April 5, 2013, and at times prior thereto, employees used a chain to lift and move the pipe fuser machine while walking along side and in front for guidance and spotting. The chain did not have an affixed marking indicating its intended limits. (b) Mountain Lake, Presidio, SF: On April 5, 2013, and at times prior thereto, employees used a wire rope bridle to lift and move the pipe fuser machine while walking along side and in front for guidance and spotting. The wire rope bridle's identification and load rating tag was not legible due to wear and deformity.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace: (Construction Reference: 1926.59) Mountain Lake, Presidio, SF: On April 5, 2013, and at times prior thereto, employees were exposed to hazardous chemicals such as, but not limited to, diesel fuel. The employer did not have the MSDS for this product on site.
