29 CFR 1926.1101(f)(1)(i): Where exposure monitoring was required under 29 CFR 1926.1101, the employer did not perform monitoring to determine accurately the airborne concentrations of asbestos to which employees were or potentially were exposed:     a. Roof - 03-18-2013 to  03/28/2013, monitoring conducted for employees removing roofing materials which contained asbestos, did not accurately reflect the 8-hour TWA (time weighted average) exposure to asbestos in that:              1. An employee, working on the roof and who was recorded as being monitored, did not wear a sample.                   2. Employees, recorded as being monitored while working on the roof, did not work on the roof or wear a sample.               3.  The asbestos competent person/supervisor could not say to whom the pump/samples were issued.             4.   It could not be determined what sampling occurred, or for what duration, if any, the sample was actually worn by a worker.              5.  The asbestos supervisor/competent person indicated that a sample may have combined a personal exposure and an area sample.      b. Boiler Room - On 03/25/2013 and 03/26/2013, an accurate determination of employee exposures, representative of the 8-hour TWA, were not made in that:            1. The sample filters were allowed to become overloaded on both days this work was performed.            2. An employee, working in this area and who was recorded as being monitored, did not wear a sample.    c. Worksite - 03/18/2013 to 03/29/2013, Collection of samples did not meet the requirements of mandatory Appendix A of this standard in that:            1. Each set of samples taken did not include 10% field blanks or a minimum of 2 field blanks as required by item 4 of the Sampling and Analytical Procedure of mandatory Appendix A. Only one blank was submitted with each set of daily samples.               2.  Each personal sampling pump was not calibrated before and after use as required by item 6 of the Sampling and Analytical Procedure of mandatory Appendix A.               3.  For samples taken during abatement of the boiler room, the appropriate levels of filter loading were not obtained and filters were not inspected to determine whether the samples might become overloaded and require a second filter as required by item 11 of the Sampling and Analytical Procedure of mandatory Appendix A.
