29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels are not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Exterior Qualities Home Improvement LTD., Napoleon, Ohio:  The employer does not assure that employees who are engaged in roofing activities are protected from falls, as observed on April 5, 2013 at the address of 1000 Westmoreland Ave., Napoleon, Ohio.  Exterior Qualities Home Improvement LTD. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 311607287, citation number 2, item number 1 and was affirmed as a final order on March 20, 2009, with respect to a workplace at 3236 Cherry St., Toledo, Ohio.  Exterior Qualities Home Improvement LTD. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA Inspection Number 313785206, citation number 2, item number 1 and was affirmed as a final order on February 13, 2012 with respect to a workplace at 465 Moline-Martin Rd., Walbridge, Ohio.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for the eyes, is not used where ever it is necessary by reason of hazards encountered in a manner capable of causing injury or impairment in the function of any part of the body through physical contact:  a) Exterior Qualities Home Improvement LTD, Napoleon, Ohio:  The employer fails to ensure that each employee uses appropriate eye protection while engaged in nailing activities, such as securing shingles to the structure roof with a pneumatic nailer, exposing employees to eye injuries from flying particles.  This violation was most recently observed on April 5, 2013 at the address of 1000 Westmoreland, Ave., Napoleon, Ohio.  Exterior Qualities Home Improvement LTD. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95(a), which was contained in OSHA Inspection Number 313785206, citation number 1, item number 1 and was affirmed as a final order on February 13, 2012, with respect to a workplace at 5465 Moline-Martin Rd., Walbridge, Ohio.
