29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  On or about April 9, 2013 during construction activities to install a water line the employer did not ensure adequate cave-in protection was used for an employee working inside a trench approximately 16 feet long, by approximately 11 and 1/2 feet wide, by approximately 8 and 1/2 feet deep in type B cohesive soil.
