This was most recently evident during the inspection of the facility located at 4912 Leavenworth St. Omaha, NE 68106, where the employer has facility maintenance employees operating a TruTest snow blower used to remove snow from the grounds without guarding the belt and pulley assembly found on the side of the engine exposing the operator to potential amputations hazards during operation.

29 CFR 1910.1200(h)(3)(i): The employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area:    The employer failed to protect employees from potential chemical hazards associated with the use of petroleum products and pesticides.  This was most recently evident during the inspection of the facility located at 4912 Leavenworth St. Omaha, NE 68106, where the employer has facility maintenance employees repairing equipment and applying pesticides without having a written HAZCOM program as a means of outlining the companys approach to prevent employees from being injured or getting ill from exposure to a chemical.  Instances of requirements failed to be met include but are not limited to:  i.    Failure to list the chemicals used on site.  ii.   Failure to identify the methods the employer will use to inform employees of the hazards associated with chemicals.
