Debris had accumulated at the top and bottom of the ladder thereby exposing employees to tripping and fall hazards when accessing and egressing the ladder.  The area at the top of the ladder was approximately 18 feet above a lower level and employees were exposed to potential trips and falls from the upper level when trying to access/egress the ladder.  The area at the bottom of the ladder had debris accumulation approximately 2 rungs high exposing employees to tripping hazards when accessing/egressing the ladder.
