29 CFR 1910.26(a)(1): "General." Specific design and construction requirements are not part of this section because of the wide variety of metals and design possibilities. However, the design shall be such as to produce a ladder without structural defects or accident hazards such as sharp edges, burrs, etc. The metal selected was not of sufficient strength to meet the test requirements, and shall be protected against corrosion unless inherently corrosion-resistant.                        Employees were accessing the ladder, of whcih, there was structual damage to the ladder.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)                  Employees were operating a table grinder, of which, the tongue guard had an opening greater than one-fourth inch.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g., closet).            Employees were working near an exit door located in the back of the plant, of which, the door did not have a "Not an Exit" sign on it.
