29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: a) Austell Wal-Mart Supercenter Store #1586 - The employer failed to ensure employees operating forklift trucks maintain a clear view of the path of travel, exposing employees to the hazard of being struck-by the load and/or forklift trucks. The operator of a Yale #6 (model GLP040SVXNUSV084 serial #C810V02727E) forklift truck was moving in the forward direction with 45 pallets that were stacked 15 high obstructed the operator's view.

29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to travel with the load trailing whenever the load obstructed forward view: a) Austell Wal-Mart Supercenter Store #1586 - The employer failed to ensure employees operating forklift trucks carrying loads that obstructed their forward view, instead travel in reverse with the load trailing, exposing employees to the hazard of being struck-by the load and/or forklift trucks. The operator of a Yale #6 forklift truck was moving in the forward direction with 45 pallets that were stacked 15 high obstructed operator's view. The Wal-Mart Store was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(n)(4), which was contained in OSHA inspection number 312795776, citation number 01, item number 001 and was affirmed as a final order on January 16, 2009, with respect to a workplace located at 501 E. Pawnee Street, Suite 100, Wichita, KS 67211.
