29 CFR 1910.37(a)(3): Exit routes were not unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. Stairs or a ramp were not provided where the exit route was not substantially level.    a) On or about 04/10/13 at the west end of the shop area; access to the south exit door was blocked by a stack of five bags of sand, digging tools, plastic and metal fence posts, and other decking materials.  A black bag of sand was found against the door on the discharge side of the exit door and the discharge area from this exit was an uneven surface.      No Abatement Certification Required

29 CFR 1910.132(a): Protective equipment was not provided whenever hazards of the environment capable of causing injury and impairment were encountered:    a) On or about 4/10/13 in the warehouse; installation and delivery trucks, some with back up alarms, are parked each night in the warehouse.  Each morning these trucks are backed out of this inside parking location through the warehouse and out the overhead door.  While backing up a 2007 Dodge Ram Pickup truck (without a back up alarm) from the inside parking location, an employee, walking from inside the warehouse crossed behind the truck at the overhead door opening and was struck.
