29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 4/4/13 at a residential new-build located at 15 Basswood Court, Orchard Park, New York; employees were installing underlayment and shingles on the roof of a 2 story house using a ladder jack scaffold. There was no personal fall arrest system in use.      NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 4/4/13 at a residential new-build located at 15 Basswood Court, Orchard Park, New York; employees were installing underlayment and shingles on the roof of a 2 story house with an 8 in 12 pitch. The was no fall protection in use.      NO ABATEMENT CERTIFICATION REQUIRED
