29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    A.)  Tim Colvin Construction, Inc., 3024 West 12th Street, Erie, PA  16505, on or about April 12, 2013 and times prior thereto:   Employees conducting wood truss framing operations were working from a metal plank at a height of 11.7 feet.  The employees were not utilizing a means or method of fall protection, including, but not limited to, guardrail systems, safety net systems, or personal fall arrest systems.

B.)  Tim Colvin Construction, Inc., 3024 West 12th Street, Erie, PA  16505, on or about April 12, 2013 and times prior thereto:   An employee's previous fall protection training was not verified by the employer, and a written certification record was not prepared.
