New structural load bearing components such as upper columns and an intermediate header were used. An operational test was not performed prior to hoisting any load exposign employees to struck by, crushing, and fall hazards from collapse. Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation that it is in compliance with the standard, including the steps that it is taking to ensure, prior to initial use, new and altered temporary overhead and gantry cranes are operationally tested to ensure compliance with 29 CFR 1910.179.

The crane was then put into service, without an inspection, to lift a stator that weighting in excess of one million pounds, exposing employees to struck by, crushing, and fall hazards.
