29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:     a) for the employee using a JLG T-350 aerial lift at 828 Providence Road Brandon (FL),  as observed on or about 04/15/2013.  The employee was removing sign letters at the southeast side of the building without a body belt or safety harness.  The employee was exposed to an 11-foot fall hazard.
