29 CFR 1926.503(c): The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:  Employees replacing the roof on a single story house located at 1945 S Smithmoor St, Wichita, KS 67207 were exposed to fall hazards in that, employees were not retrained on the changes to the type of fall protection acceptable for residential construction.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Employees replacing the roof on a single story house at 1945 S Smithmoor St, Wichita, KS 67207 were exposed to fall hazards of 10 feet eave to ground in that, no means of fall protection was used.  The pitch of the roof was approximately 5 in 12.   Mateo Gasca was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 314030917, citation number 1, item number 2 and was affirmed as a final order on December 1, 2010, with respect to a workplace located at 2102 S Crestline St, Wichita, KS, 67209.
