29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) 1601 North Front Street Harrisburg, PA - Employees were observed using an Aluminum Extension Ladder that did not extend at least 3 feet above the shed type roof landing at the side entrance to the building, thereby exposing employees to a fall hazard of approximately 14 feet to the ground below, on or about April 15, 2013. (b) 1601 North Front Street Harrisburg, PA - Employee(s) were observed using an Aluminum Ladder from the shed roof at the side entrance accessing the main roof that did not extend at least 3 above the landing, thereby exposing employee(s) to a fall hazard of approximately 9 feet to the shed roof and approximately 25 feet from the main roof to the the ground below, on or about April 15, 2013. No abatement certification or documentation required.
