29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:       a) On or about April 15, 2013, at the above located facility, an employee was required to work with sodium hypochlorite and a cyanide plating solution (Alkaline chlorination process). The employer did not ensure that the employee used a splash proof apron while working with the chemicals. The employee was thereby exposed to the hazards associated with chemical contact.

The employer did not verify that the required workplace hazard assessment had been performed through a writtn certication.
