On the northeast side, there was a 5 feet wide opening without guardrails.

29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level: a) In the Small Warehouse, at the upstairs work area, employees were exposed to fall hazards of 8 feet due to the lack of a complete guardrail system around the overhead work area. On the south, west and west section of the north guardrails, the top rail was 36 inches high and the midrail was missing from the guardrails.

Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a) Big Warehouse, on the west side, the 110 volt electrical receptacle was missing its faceplate, exposing the inner electrical wiring connections.
