29 CFR 1910.178(l)(6):  The employer did not certify that each operator has been trained and evaluated in the safe operation of the forklifts in the workplace:       (a)Bloomin Flower Cards at 3080 Valmont Road, Boulder, CO 80301: On and before April 17th, 2013, the employer did not certify that the forklift operators had been trained and evaluated as required by 29 CFR 1910.178(l) in the topics as outlined in 29 CFR 1910.178(l)(3).       Abatement Note:  The certification shall include:       (1)  The name of the operator;   (2)  The date of the training;   (3)  The date of the evaluation; and,   (4)  The identity of the person(s) performing the training and evaluation.

This condition exposed employees to amputation and crush hazards. Some instances include, but are not limited to:      1.Brandtjen & Kluge Automatic D Series Presses   2.Chandler letter presses

29 CFR 1904.40(a):  The employer did not provide an authorized government representative the records kept under Part 1904 within the four business hours:     (a)Bloomin Flower Cards at 3080 Valmont Road, Boulder, CO 80301: On and before April 17th, 2013, the employer did not ensure to provide copies of the injury and illness records to an authorized representative, in that, OSHA 300 and 300A reports were requested for 2011 through current and were not received within four business hours.     Abatement Note: Not maintaining injury and illness records does not ensure that the documents are reviewed annually by the employer to be in compliance with 29 CFR 1904 and inhibits identification of injury and illness trends located at the worksite.  Not providing records to government representatives within 4 business hours prevents the representative from ensuring that injuries and illnesses are being recorded appropriately to be in compliance with 29 CFR 1904.

29 CFR 1910.157(e)(3):  Portable fire extinguishers were not subjected to an annual maintenance check:         (a)Bloomin Flower Cards at 3080 Valmont Road, Boulder, CO 80301: On and before April 17th, 2013, the employer did not ensure all portable fire extinguishers were subjected to an annual maintenance check in that all fire extinguishers located in the shop area had not receive its annual service in over a year.  This condition exposed the employees to the hazards associated with fire and smoke.
