29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power presses:  At the time of the inspection, mechanical power presses #3, 11,and 12 were operated without guards to prevent employees from exposure to the point of operation amputation hazards.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1904.32(a): 29 CFR 1904.32(a):  The employer did not ensure that at the end of the year that entries were complete and accurate; and an annual summary of injuries and illnesses on the OSHA 300 Log was created, certified and posted.       At the time of the inspection, the OSHA 300A for 2011 and 2012 were not complete, certified or posted as required.
