29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided. (a)Guzeven Construction, Inc. @ 208 Darlington Pl, Johnstown, CO 80534: On and before January 15th, 2013, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in sheathing operations while working at the unprotected edges on the second story of a multi-family home under construction. Employees were not utilizing a fall protective system. Employees were exposed to fall hazard of approximately 10'.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) Guzeven Construction, Inc. @ 208 Darlington Pl, Johnstown, CO 80534: On and before April 15th, 2013, the exposing employer did not ensure that each employee engaged in sheathing operations on a new multi-family residence, had been trained to recognize the hazards associated with working at elevations over 6'. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees working on-site had not received fall protection training before working at elevations over 6 feet. The condition exposed employees to the hazard of falling from an elevation of approximately 10 feet.
